Contact PALS by telephone at 01392 675 686 or email dpt.pals@nhs.net. Opening hours are Monday to Friday, 10am to 4pm (closed bank holidays).
Your views are important to us. We want to hear about your experiences and there are several ways you can let us know what you think about our services.
If you have a compliment, concern or complaint about the service you have received, you can contact PALS by telephone at 01392 675 686, email dpt.pals@nhs.net or by completing the forms below. Opening hours are Monday to Friday, 10am to 4pm (closed bank holidays).
Alternatively, talking to someone involved in your care may help to resolve things quickly.
We can only investigate complaints about services provided or funded by Devon Partnership NHS Trust. If you are taking legal action about your complaint, we will need to discuss this with you.
To investigate your concerns, we need as much information as possible, such as dates, times and the names of the people involved.
We want to hear about your experience - whether good or bad - so we can continue improving out services.
Because the care and treatment we provide is so diverse, we have designed service-specific surveys to help us understand how we're doing. These also include the nationally required Friends and Family Test questions.
We keep records about your health and any treatment and care you receive to ensure you get the best care from us.
You have the right to see the information we hold about you, including paper records and those held on a computer. There are exceptions, such as if disclosing information may cause serious harm to you or others, or if it includes information from a third party without their consent.
To request a copy of your records, contact the Information Governance Team by telephone: 01392 675678 email: dpt.ig@nhs.net or letter:

We will provide your records as quickly as possible, and always within 40 days of receiving your completed application. A payment towards our costs may be required, but it will not be more than £50.
The Freedom of Information (FOI) Act gives you the right to access recorded information held by the trust. It aims to increase openness and transparency for the services we provide.
We routinely publish information about our organisation, including declarations, reports and expenditure.
You can submit an FOI request by email to: dpt.ig@nhs.net, or by letter to:
