This is not a comprehensive list but are the most common issues:
- We know that some of our documents are not fully accessible, but we are required to publish them to meet legal or statutory regulations. We have published all our documents in one place on our Publications page
- Many of our pdfs are not accessible. For example, they may be missing alternative texts for images and graphics, as well as document structure. - Some pages scroll in two dimensions on small mobile screens, WCAG 2.1 AA 1.4.10
- We need to fix duplicate IDs. WCAG 2.0 A 4.1.1
- Some of our links need to explain their purpose and how they open, for example, in a new tab. WCAG 2.0 AAA 3.2.5, WCAG 2.0 A 2.4.4
- Some functionality in News needs to be reviewed for user control and flash speed. WCAG 2.0 A 2.3.1
- Some images require more appropriate alternative text. WCAG 2.0 A 1.1.
We are aware that the following referral forms are not fully accessible, but have published the documents for the benefit of patients, whilst we work on creating accessible versions:

What we are doing to address non-compliance.
- We have purchased Silktide which we will use for continuous monitoring for accessibility on the website.
- The Trust's Communications team has overall responsibility for running accessibility reports and fixing issues.
- We are developing a plan to review our published documents and to create content as html pages or fully accessible pdfs or word documents.
- When policies and procedures are due to be reviewed we will take that opportunity to redesign and publish fully accessible content.
